Why does a cough get worse at night?

7 reasons few people think of — but that matter

Nighttime coughing is one of the most unpleasant experiences.
During the day we cough and keep moving.
But in the evening, when everything goes quiet… that’s exactly when the cough wakes up.

Many parents say:
“During the day they barely cough, but once they lie down — it doesn’t stop.”
And it’s true.
But the reasons often remain unknown.

The throat becomes drier at night

When we sleep, we start to:
• swallow less often
• moisten the throat less
• breathe through the mouth if the nose is blocked

The result?
The mucosa dries out — and dry mucosa is easily irritated.
This is the most common “silent” culprit behind nighttime coughing fits.

Lying down changes everything

When we lie down:
• mucus collects more easily at the back of the throat
• it drains more slowly
• it creates a “tickle” sensation that triggers the cough reflex

A child may sleep calmly, but the body is trying to clear what has built up.

Mucociliary clearance slows down at night

This is the body’s natural mechanism that “pushes” mucus outward.
At night it:
• works more slowly
• is less effective
• can’t move mucus quickly

And the result is…
coughing with the feeling that “something is stuck.”

Dry room air increases irritation

Especially in winter.
Heating:
• dries the air
• dries the mucosa
• makes the throat more sensitive

In children, the effect is even stronger because their mucosa is thinner.

Throat nerve receptors are more sensitive in the evening

This is a little-known fact.
Late in the evening:
• the nervous system shifts rhythm
• throat receptors become more reactive
• they perceive irritation more strongly

That’s why the cough can seem “deeper” and “louder.”

Reflux can trigger coughing… even without heartburn

Many parents are surprised, but:
With reflux, a small amount of acid or stomach contents can reach:
• the back of the throat
• the vocal cords

…and cause a dry, persistent cough that worsens at night.
And often there is neither pain nor heartburn.
